Clinical Fellowship (Nephrology) University Of Toronto, Canada

Clinical Fellowship Program in Nephrology in University of Toronto is one of the largest nephrology programs in North America. It incorporates rotation at 4 different hospitals–Toronto General Hospital, Toronto Western Hospital, Sunnybrook Health Sciences center and St. Michael Hospital and also covers Princess Margaret Cancer Hospital and Mount Sinai Hospital.

The Division of Nephrology at UHN participates in the investigation and treatment of all types of renal disease, encompassing treatment of Chronic Kidney Disease (CKD) with dialysis and transplantation, general nephrology, subspecialty clinics, teaching and research

In-patient clinical services consist of the Nephrology Ward, and the Multiorgan Transplant Unit. Outpatient clinical services consist of Home Peritoneal Dialysis, Intermittent Peritoneal Dialysis, Outpatient Hemodialysis, Home Hemodialysis, Self-Care Hemodialysis, and Rehabilitation and Complex Continuing Care dialysis, Nocturnal Hemodialysis and out-patient clinics.

This 2 year fellowship involves rotation at all these 4 hospitals and also through all subdivisions like Hemodialysis, Ward, Consult services, peritoneal dialysis, Kidney Transplant Unit and Home hemodialysis. While rotating through these sub divisions we undergo training under world renowned Nephrologist and achieve tremendous experience in their field.

These 2-1/2 Yrs years of fellowship provided me an exciting experience in diverse areas of Nephrology. Like advanced experience in various modalities of acute and chronic renal replacement therapy, access management, intensive care Nephrology, advanced aspects of fluid, electrolyte and acid-base management and consultative Nephrology. I was rotated in different major fully affiliated teaching hospitals of the University of Toronto: The University Health Network (which includes - Toronto General Hospital and Toronto Western Hospital and also covers Princess Margaret cancer Hospital and Mount Sinai Hospital) and Sunnybrook & Women's College Health Sciences Centre (Sunnybrook Campus). There is a comprehensive seminar series for the trainee, in addition to a regular rounds schedule

Peritoneal dialysis : I got 4 months of rotation in Home peritoneal dialysis. Wehave 170 patients active follow up in HPDU. I worked with Prof DimitriosOreopoulus and Prof Joanne Bargman who are world renowned Nephrologists in the field of Peritoneal dialysis.

Hemodialysis : I had 7 months of rotation in Hemodialysis including 2 months in HomeHemodialysis. We have approximately 325 patients on conventional in center and approximately 100 patients on home hemodialysis. During this rotation I worked with Prof Robert Richardson, Prof Stanley Fenton and Dr C Chan.

Renal Transplant: I had 7 months of transplant rotation. We have a multiorgantransplant unit, which does lung, liver, heart, and pancreas-kidney transplant. We have done approximately 150-kidney transplant in a year with half of them as cadaver kidney transplant. Here I worked with Prof Edward Cole, D cattran, C Cardella.

Ward/Consult services: I had 10 months of rotation equally divided into Wardand Consult posting. In Consult posting, we managed all those patients admitted under other specialties and in ICUs. I got good experience of putting dialysis access, starting CVVHD and SLED. In ward, we used to manage mostly ESRD patient with access complications.

WORKSHOP ON CADEVERIC KIDNEY TRANSPLANT : Being Organizing Secretary, organized this workshop in March 2011 with eminent speakers from Mumbai, Delhi.
